Liver biopsy is rarely needed for Hepatitis C evaluation. Non-invasive tests like FibroScan (ultrasound elastography) or blood-based fibrosis markers accurately assess liver scarring without biopsy. These tests are painless, quick, and provide immediate results. Biopsy is reserved for cases where non-invasive testing is inconclusive or when other liver conditions need evaluation. Most patients complete their entire Hepatitis C evaluation and treatment without ever needing a biopsy.

Hepatitis C spreads through blood-to-blood contact, not through casual contact, sharing food, hugging, or kissing. Household transmission risk is very low. Don't share personal items that might have blood on them (razors, toothbrushes, nail clippers). Sexual transmission is uncommon in monogamous relationships but increases with multiple partners or rough sexual practices that cause bleeding. Once you're cured, you cannot transmit the virus. Family members don't need testing unless they have independent risk factors.

Family members should be tested if they have independent risk factors: blood transfusion before 1992, injection drug use, birth between 1945-1965 (baby boomer cohort), or known exposure to infected blood. Hepatitis C doesn't run in families genetically—it's an infectious disease, not an inherited condition. The CDC now recommends universal screening for all adults at least once, so family members should discuss screening with their primary care doctors regardless of your diagnosis.

True treatment failure (virus not eliminated) is rare with modern direct-acting antivirals—cure rates exceed 95%. If the virus is detected after treatment, it's usually either incomplete viral clearance (detected during treatment monitoring) or reinfection from new exposure. Incomplete clearance can be retreated with alternative regimens. Reinfection requires new exposure to infected blood. Once cured, you don't have immunity—reinfection is possible if exposed again, but this is uncommon in most patients.

Cure is confirmed by sustained virologic response testing (SVR12)—undetectable HCV RNA 12 weeks after completing treatment. Your hepatologist will check viral load during treatment (usually week 4) to confirm the medication is working, then again 12 weeks after your last dose. If the virus remains undetectable at SVR12, you're cured. This test timing is standard because late relapse after SVR12 is extremely rare. Most patients know they're cured within 5-6 months of starting treatment.
